Root Cause Analysis&nbsp;<\/h3>\n<p>When you launch rockets or fly airplanes full of passengers, you want things to go well. You don&rsquo;t want <em>mishaps<\/em>.<\/p>\n<p>There is an engineering discipline called <a href=\"https:\/\/en.wikipedia.org\/wiki\/Root_cause_analysis\">r<em>oot cause analysis<\/em><\/a>&nbsp;dedicated to providing objective techniques for trying to make sure bad days don&rsquo;t happen. It is used by NASA and the FAA &mdash; think the <a href=\"https:\/\/en.wikipedia.org\/wiki\/Space_Shuttle_Challenger_disaster\">Challenger explosion<\/a>, think near collisions between <a href=\"https:\/\/www.npr.org\/2023\/02\/04\/1154549311\/near-collision-fed-ex-southwest-plane-faa\">passenger jets on a runway<\/a>. But it&rsquo;s actually a general tool for thinking about how to avoid problems.<\/p>\n<p>Today you will use <em>root cause analysis<\/em> as a tool to understand the US Civil War. And you&rsquo;ll also do some math.<\/p>\n<h3>2. The Assignment<\/h3>\n<p>I hand out the assignment to the class. It includes what the students are to do and a rubric for how I will grade their work. Here are their instructions.<\/p>\n<ul>\n<li>Write a brief definition of <em>root cause analysis<\/em>. Explain what it is and what some of the core concepts are. Cite your references.<\/li>\n<li>Provide two examples of how <em>root cause analysis<\/em> has been used in the real world. Briefly explain each example and how the analysis was used. Cite your references.<\/li>\n<li>Briefly explain your understanding of the difference between a <em>root cause<\/em> and a <em>proximate cause<\/em>.<\/li>\n<li>Before 2019, the Texas <a href=\"https:\/\/www.npr.org\/2018\/11\/16\/668557179\/texas-students-will-soon-learn-slavery-played-a-central-role-in-the-civil-war\">social studies curriculum mandated<\/a> teaching the causes of the civil war as (a) sectionalism, (b) states&rsquo; rights, and (c) slavery, in that order. Briefly summarize what each of those causes &ldquo;mean&rdquo; with regard to the US Civil War. Cite your references.<\/li>\n<li>Explicitly classify those three causes of the Civil War as either <em>root<\/em> or <em>proximate<\/em> causes. Briefly justify your classification based on the definitions.<\/li>\n<li>Briefly explain why you do or don&rsquo;t feel that (a) the US Civil War was a &ldquo;mishap&rdquo; and (b)&nbsp;<em>root cause analysis<\/em> is a useful tool for understanding the history of the US Civil War. (There is no &ldquo;correct&#8221; answer, here. I am interested in your thinking.)<\/li>\n<li>And&#8230; since this is a math class, also complete this <a href=\"https:\/\/www.longbranch.k12.nj.us\/cms\/lib\/NJ01001766\/Centricity\/Domain\/1493\/Synthetic%20Division%20Worksheet.pdf\">synthetic division worksheet<\/a>.<\/li>\n<\/ul>\n<p>Let&rsquo;s skip over the grading rubric.
